I ran across a few of these while searching for some info...

 

Those of you who have worked in shops will either cringe or laugh or both...

 

Most likely someone forgot that negative signs and decimal points are very important in G-Code... The boss will be disapproving.

The engineers sometimes brain fart. I have to admit not seeing this even though it's highlighted.

Not the worst I've seen. Most set-up guys wouldn't catch that until they ran a sample part. Then wonder why the holes didn't get threaded.
As to the smashed holder. I saw a guy load a magazine with each tool shifted one position over. Then didn't discover it until he wiped out six tools. Sad part in your photo is he not only ruined a wheel that was mostly done, but also wasted a lot of machine time. My old boss at CWM would have been more upset about that than the ruined wheel.
