Who made this HO slot car? What year was it made? What car is it modeled after? It looks smaller then the other HO cars I have...

All I am sure of that it is a slot car and that it has Continental on the tires.

If anyone can help me identify this car, I would be truly grateful.

I'm not an HO guy, so I'm really out on a limb here, but based on the headlight cover shape and the louvers in the big rear window, I'm going to guess that the car being modeled is a Porsche 904.

And I think the maker is Faller of Germany, but have no idea of the year of manufacture.

Never forget that first place goes to the racer with the MOST laps, not the racer with the FASTEST lap

It's an Atlas Zinger Porsche 904 from the mid-1960s.

Indeed. They came in racing sets but also in kits under blisters on a small card... :)
