Does the outside diameter of the gear matter? If I have two 42t gears but each has a different OD. Are they different ratios?

No.
A motor with say , a 14T pinion, still has to rotate 3 times to get the spur to rotate 1 time.

Two gears, with the same teeth, advertised as the same pitch, will vary in diameter.
I have an old Koford 64p 37t, and it's about .003" or .004" bigger than a JK 64p 37t.
Edit: looking at a Camen/Cahoza 64p,37t it's about .005" bigger than the JK.
