Great progress.
What are you doing for a heatsink?
I think:
1. The brake circuit carries the full back EMF of the motor so it should have thick wires for optimal performance (especially if the relay fails). I guess thin wire could be considered part of the resistance network.
2. The power traces need to be as wide as possible and the wires need to connect directly to the relay and transistors. You don't want power to be travelling along PCB traces if possible.
I would only install one output transistor for testing.
